UPPSC DV FORMSET RELEASED:

Here is good news for those candidates who appeared in the Staff Nurse/ Sister Grade-II exam 2019 which was conducted by UPPSC. As per the new provisional selection result of Staff Nurse/Sister Grade-2 (Male/Female), Examination-2021 released by the commission on August 23, 2022, there are 146 newly selected candidates in the selection result who were not included in the earlier selection result released on 04.01.2022 earlier. In addition to the above, there are 104 such candidates, who have not got their Documents Verified by appearing in the Commission's office in the past. The commission will conduct the Document Verification of those 250 candidates who have been selected for the document verification. So they ought to be prepared for document verification by this point. The UPPSC has activated the DV Formset link. After document verification, appointments will only be made on a merit basis. The commission will soon announce the document verification date.

How to Download UPPSC Staff Nurse / Sister Grade-II DV Formset

Visit the UPPSC website at www.uppsc.up.nic.in as the first step.

• On the homepage, click the link that indicates "CLICK HERE TO DOWNLOAD DOCUMENT VERIFICATION FORMSET FOR STAFF NURSE/SISTER GRADE-2 (MALE/FEMALE) IN ADVT.NO. A-4/E-1/2021, U.P. MEDICAL AND HEALTH SERVICES DEPTT./MEDICAL EDUCATION AND TRAINING DEPTT./K.G.M.U.l"  under the Activity Dashboard.

• Now print the previously filled online formset (Application Form, Authentication-Form, Desh-Form and Marksheet Details) Along with preparing a set of self-attested copies of all the educational records/certificates, by signing on each page of that form set at the designated place and with two copies of the photo attested by the gazetted officer.

• Make sure candidates have to reach the commission premises Prayagraj, by entering from gate number-3, Saraswati Bhawan. 

At the time of verification, submit the original and self-attested photocopies of the following records/certificates along with the application form:-

1- Mark sheet and certificate of high school examination and intermediate examination with science from the Board of Secondary Education, Uttar Pradesh.

2- Diploma in General Medical and Obstetrics Training eligible for registration in Uttar Pradesh Nurses and Maternity Council or Bachelor's (B.Sc) degree in Nursing or Diploma in Psychiatry for male candidates.

3- Certificate of registration as a nurse and midwife from Uttar Pradesh Nurses and Midwives Council.

4- Caste certificate on the updated prescribed format regarding the reservation (the certificate should not be more than six months old in respect of the Scheduled Tribes Gond and Nayak of Uttar Pradesh).

In the case of a Government servant/personnel, no objection certificate in original given by the employer.

7- Caste certificate regarding reservation issued by father's side for female candidates.

8- Experience certificate issued by the competent authority as per the standard by the

staff nurse (male/female) working on a contract basis under the Government of Uttar Pradesh. 9- If it is found from the certificates/records submitted by the candidate that he/she does not possess the advertised qualification for the post of Staff Nurse (Male/Female) till 03.09.2021, the last date of advertisement, then his/her candidature will be cancelled.
